In SIP releases 4.2 and later, an Unlock Config item displays in the phone settings menu. When you select Unlock Config, you are prompted to enter a phone password. When the Network Configuration or SIP Configuration menus display, the lock icon in the upper-right corner of the LCD indicates an unlocked state. The unlocked symbol indicates that you can modify the network and SIP configuration settings.The phone automatically relocks the configuration.

To unlock the Network Configuration and reset to factory, the Cisco method is to hit the Settings button, press **#, and that unlocks the Network config. Then, select option 3 (Network Configuration), then scroll down to option 33 (Erase Configuration), and select Yes. This should reset the phone back to factory default.

Another Idea is

Fastest way to wip everything out
1. Power off the phone completely.
2. Hold down the # key, then power the phone on.
3. Continue holding the # key until the LCD gives you a status message.
4. Follow the prompts to do a full factory reset, which resets the password as well.
